Health Insurance Costs: Projected Increase to Be the Largest in 15 Years

Employers are bracing for 2026 health insurance cost increases projected to be the largest in 15 years. The surge in health benefit expenses could place financial pressure on organizations that aren’t actively planning cost-reduction strategies.

Projections

A new Mercer report projects that the total health benefit cost per employee will rise 6.5% on average in 2026, the highest increase since 2010. For comparison, Mercer reported a 3% annual increase over the past decade.

Employers not making any plan changes for 2026 could experience a 9% increase. Mercer expects employees to pay, on average, an additional 6% to 7% in premiums. Notably, some organizations may try to limit the increase by raising deductibles and copayments instead.

The Cause of Cost Increases

While increased costs are an effect of higher utilization, surveyed employers attribute the 2026 increase to several additional factors, including:

  • More cancer diagnoses and the escalating cost of treatment
  • Overall rising pharmacy costs, especially the increased price and utilization of GLP-1s for weight loss
  • Growing cases of chronic and complex conditions
  • Expansion of mental health services and access and reduced stigma
